Enrollment Application
Enrollment in UHM SEED Academy at Kapolei High School is easy.
Step 1: Register
If you have not already done so, register online to save your spot in UHM SEED Academy at Kapolei High School.
Step 2: Enroll
Once registered, fill out and print the Enrollment Application [521 KB] and submit to UHM SEED Academy at Kapolei High School by fax, mail, or email.*

Part 1: Pages supplied in the Enrollment Application packet
- Student Enrollment Form
- Release of Student Records
If your student has been home schooled prior to enrolling in UHM Seed Academy, please call us about submitting alternative documentation.
- Epi-Pen/Inhaler Consent Form
If self-administration of medication is required
- Student's Health Record
Part 2: Items you will need to supply
- Birth Certificate
Copy of the Birth Certificate, Passport or visa if the student is from a foreign country
- Cumulative Transcripts from all high schools attended
If your student has been home schooled prior to enrolling in UHM SEED Academy, please call us about submitting alternative documentation.
- Current Proof of Address
Copy of rental or mortgage document, current utility bill, notice of base housing assignment, or notarized statement of residence if living with relative.
- Power of Attorney
If not living with parent.
- Special Education Records
If applicable: 504/IEP, Evaluation (MET) Report, and any other Special Education Records (psychological evaluations, etc.)

Once your Enrollment Application is processed, an UHM SEED Academy academic advisor will contact you to help you select your courses.
